The interaction of these dimensions (<\/span><b style=\"font-size: 1rem;text-align: initial\">sensory-discriminative:<\/b><span style=\"text-align: initial\"><span style=\"font-size: 1rem\">\u00a0intensity, location, quality and <\/span>behavior<span style=\"font-size: 1rem\">\u00a0of pain;\u00a0<\/span><\/span><b style=\"font-size: 1rem;text-align: initial\">cognitive-evaluative:<\/b><span style=\"font-size: 1rem;text-align: initial\">\u00a0thoughts of the pain as influenced by previous experiences and knowledge; and\u00a0<\/span><b style=\"font-size: 1rem;text-align: initial\">motivational-affective<\/b><span style=\"font-size: 1rem;text-align: initial\">: emotional responses like anger, anxiety and fear that motivate the response to pain)<\/span><sup id=\"cite_ref-Gifford_and_Butler_1-0\" class=\"reference\" style=\"text-align: initial\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-Gifford_and_Butler-1\">[1]<\/a><\/sup><span style=\"font-size: 1rem;text-align: initial\">\u00a0all contribute to the complexity of the painful experience. An individual\u2019s belief systems, pain understanding, thoughts and emotions: anxiety; depression; catastrophizing (or their psychology) influence how the brain interprets a noxious stimulus in relation to the meaning and level of threat the stimulus pose to one's well-being, and thereby influence the resulting output from the brain<\/span><sup id=\"cite_ref-2\" class=\"reference\" style=\"text-align: initial\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-2\">[2]<\/a><\/sup><span style=\"text-align: initial\"><span style=\"font-size: 1rem\">. Such resulting changes in <\/span>behavior<span style=\"font-size: 1rem\">\u00a0(i.e. avoidance)<\/span><\/span><sup id=\"cite_ref-Moseley_3-0\" class=\"reference\" style=\"text-align: initial\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-Moseley-3\">[3]<\/a><\/sup><sup id=\"cite_ref-Turk_and_Okifugi_4-0\" class=\"reference\" style=\"text-align: initial\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-Turk_and_Okifugi-4\">[4]<\/a><\/sup><span style=\"font-size: 1rem;text-align: initial\">\u00a0such as fear avoidance, catastrophizing, pain-related anxiety, stress and learned helplessness can impact upon a pain experience and a patient's coping strategies. While this is unlikely to be an exclusive list of influential psychosocial factors, the following components are well researched and evidenced. This page will review each of these in turn.<\/span><\/div>\r\n<\/div>\r\n<h2><span id=\"Fear_Avoidance\" class=\"mw-headline\">Fear Avoidance<\/span><\/h2>\r\n<a class=\"mw-redirect\" title=\"Psychological approaches to pain management\" href=\"https:\/\/physio-pedia.com\/Psychological_approaches_to_pain_management\"><b>Pain and Fear<\/b><\/a>: Fear is a typical emotional response to an experience that is perceived to be threatening, such as one that leads to a pain sensation<sup id=\"cite_ref-Rainville_et_al_5-0\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-Rainville_et_al-5\">[5]<\/a><\/sup><sup id=\"cite_ref-Leeuw_et_al_6-0\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-Leeuw_et_al-6\">[6]<\/a><\/sup>.\u00a0Future avoidance of the painful activity is often the resulting behaviour\u00a0adopted to protect oneself from a repeat of the fearful emotion and painful experience. This pattern can lead to fear and avoidance of work-related activities, movement, and re-injury<sup id=\"cite_ref-Vlaeyen_and_Linton_7-0\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-Vlaeyen_and_Linton-7\">[7]<\/a><\/sup>.\u00a0<i><b>Kinesophobia<\/b><\/i>\u00a0is a term used interchangeably with fear avoidance in health and pain literature. It is defined as \u201can excessive, irrational, and debilitating fear of physical movement and activity resulting from a feeling of vulnerability to painful injury or reinjury<sup id=\"cite_ref-Kori_et_al_8-0\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-Kori_et_al-8\">[8]<\/a><\/sup>.\u201d\r\n<h3><span id=\"Impact_of_Fear_Avoidance_on_the_Pain_Experience\" class=\"mw-headline\"><b>Impact of Fear Avoidance on the Pain Experience<\/b><\/span><\/h3>\r\n<div class=\"thumb tright thumbnail pull-right\">\r\n<div class=\"thumbinner\"><a class=\"image\" href=\"https:\/\/physio-pedia.com\/File:Fear-avoidance-picture.jpg\"><img class=\"thumbimage img-responsive aligncenter\" src=\"https:\/\/physio-pedia.com\/images\/thumb\/2\/27\/Fear-avoidance-picture.jpg\/300px-Fear-avoidance-picture.jpg\" alt=\"Fear-avoidance-picture.jpg\" width=\"362\" height=\"188\" \/><\/a>\r\n<div class=\"thumbcaption caption\">\r\n<div class=\"magnify\"><\/div>\r\n<\/div>\r\n<\/div>\r\n<\/div>\r\nIn response to acute tissue damage\/injury, feelings of fear are heightened as part of a normal consequence of pain<sup id=\"cite_ref-Turk_and_Okifugi_4-1\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-Turk_and_Okifugi-4\">[4]<\/a><\/sup>. The individual will rest and protect the painful area as an adaptive behaviour to allow tissue healing to occur<sup id=\"cite_ref-Turk_and_Okifugi_4-2\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-Turk_and_Okifugi-4\">[4]<\/a><\/sup>. As the acute phase and initial tissue healing resolves, it has been suggested that some individuals can confront their fearful emotions and are able to resume normal activities, which ultimately helps extinguish their fears as they experience positive increase in mobility\/movement unassociated with further increases in pain<sup id=\"cite_ref-Rainville_et_al_5-1\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-Rainville_et_al-5\">[5]<\/a><\/sup>. For others, they may be unable to\u00a0<b>overcome the fearful emotion<\/b>\u00a0and the resulting\u00a0<b>avoidance behaviour persists<\/b><sup id=\"cite_ref-Vlaeyen_and_Linton_7-1\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-Vlaeyen_and_Linton-7\">[7]<\/a><\/sup>. A cycle of continued activity avoidance and fearful emotions may ensue, resulting in chronic pain that may have an adverse effect on the musculoskeletal and cardiovascular system (disuse syndrome)<sup id=\"cite_ref-Bortz_9-0\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-Bortz-9\">[9]<\/a><\/sup>, which fundamentally increases disability<sup id=\"cite_ref-Turk_and_Okifugi_4-3\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-Turk_and_Okifugi-4\">[4]<\/a><\/sup><sup id=\"cite_ref-Vlaeyen_and_Linton_7-2\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-Vlaeyen_and_Linton-7\">[7]<\/a><\/sup><sup id=\"cite_ref-10\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-10\">[10]<\/a><\/sup>.\r\n\r\n<b>Several models have been proposed to explain the persistence of 'fear-avoidance'<\/b>\u00a0<b>and its impact on pain and behaviour<\/b>.\r\n\r\nIn the\u00a0<b><a title=\"Fear Avoidance Model\" href=\"https:\/\/physio-pedia.com\/Fear_Avoidance_Model\">fear-avoidance model of pain<\/a><\/b>, individuals who are likely to demonstrate ongoing fear-avoidance behaviours will be inclined to\u00a0<b>catastrophize<\/b><sup id=\"cite_ref-Leeuw_et_al_6-1\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-Leeuw_et_al-6\">[6]<\/a><\/sup><sup id=\"cite_ref-Vlaeyen_and_Linton_7-3\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-Vlaeyen_and_Linton-7\">[7]<\/a><\/sup>:\r\n<div class=\"center\">\r\n<div class=\"thumb tnone thumbnail center-block\">\r\n<div class=\"thumbinner\"><a class=\"image\" href=\"https:\/\/physio-pedia.com\/File:Pain_fear.jpg\"><img class=\"thumbimage img-responsive aligncenter\" src=\"https:\/\/physio-pedia.com\/images\/thumb\/d\/d3\/Pain_fear.jpg\/300px-Pain_fear.jpg\" alt=\"Pain fear.jpg\" width=\"398\" height=\"245\" \/><\/a>\r\n<div class=\"thumbcaption caption\">\r\n<div class=\"magnify\"><\/div>\r\n<\/div>\r\n<\/div>\r\n<\/div>\r\n<\/div>\r\nVlaeyen and Linton<sup id=\"cite_ref-Vlaeyen_and_Linton_7-4\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-Vlaeyen_and_Linton-7\">[7]<\/a><\/sup>\u00a0have described the interconnections contributing to fear avoidance as follows: with negative appraisals of a painful experience, fear avoidance behaviours are elicited. Due to hypervigilance, the avoidance behaviour begins to occur in anticipation rather than in response to pain. As there are fewer and fewer opportunities to correct the wrongful association of pain with the avoided activity, the behaviour perpetuates and fear avoidance persists.\r\n\r\nWhile research has consistently shown an association between fear avoidance behavior and pain-related fears<sup id=\"cite_ref-11\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-11\">[11]<\/a><\/sup><sup id=\"cite_ref-Boersma_and_Linton_12-0\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-Boersma_and_Linton-12\">[12]<\/a><\/sup><sup id=\"cite_ref-13\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-13\">[13]<\/a><\/sup><sup id=\"cite_ref-14\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-14\">[14]<\/a><\/sup><sup id=\"cite_ref-15\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-15\">[15]<\/a><\/sup>\u00a0even when pain intensity and other variables were controlled in patients with chronic pain, there are conflicting studies about\u00a0this association in patients with acute pain.\u00a0<b>Studies have shown that high levels of fear avoidance<\/b>\u00a0can be\u00a0<b>a predictor for future episodes of pain<\/b><sup id=\"cite_ref-16\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-16\">[16]<\/a><\/sup><sup id=\"cite_ref-17\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-17\">[17]<\/a><\/sup><sup id=\"cite_ref-18\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-18\">[18]<\/a><\/sup><sup id=\"cite_ref-19\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-19\">[19]<\/a><\/sup><sup id=\"cite_ref-20\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-20\">[20]<\/a><\/sup>. But on the contrary, studies<sup id=\"cite_ref-21\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-21\">[21]<\/a><\/sup>\u00a0showed more support for the link between\u00a0<a title=\"Depression\" href=\"https:\/\/physio-pedia.com\/Depression\">depression<\/a>\u00a0with poor function and disability at this early stage<sup id=\"cite_ref-Pincus_et_al_22-0\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-Pincus_et_al-22\">[22]<\/a><\/sup><sup id=\"cite_ref-23\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-23\">[23]<\/a><\/sup>\u00a0In the\u00a0<b>learning pathway model,<\/b>\u00a0fear avoidance becomes a conditioned response through association of pain with movement<sup id=\"cite_ref-Vlaeyen_and_Linton_7-5\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-Vlaeyen_and_Linton-7\">[7]<\/a><\/sup><sup id=\"cite_ref-Rainville_et_al_5-2\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-Rainville_et_al-5\">[5]<\/a><\/sup><sup id=\"cite_ref-Pincus_et_al_22-1\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-Pincus_et_al-22\">[22]<\/a><\/sup>. The behavior of avoiding expected pain-provoking movement or activities to prevent new episodes of pain is learned through experience<sup id=\"cite_ref-24\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-24\">[24]<\/a><\/sup>. The individual may anticipate similar experiences and associate non-painful or non-harmful experiences with pain, resulting in persistent fear-avoidance behavior<sup id=\"cite_ref-Vlaeyen_and_Linton_7-6\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-Vlaeyen_and_Linton-7\">[7]<\/a><\/sup><sup id=\"cite_ref-Pincus_et_al_22-2\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-Pincus_et_al-22\">[22]<\/a><\/sup>.\r\n\r\nThe following\u00a0<b>factors<\/b>\u00a0can affect\u00a0<b>the strength of association of pain with movement and the fear-avoidance behaviour<\/b>\u00a0that patients may exhibit:<sup id=\"cite_ref-25\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-25\">[25]<\/a><\/sup>\r\n<ul>\r\n \t<li>existing beliefs about pain and the meaning of the symptoms<\/li>\r\n \t<li>beliefs about their own ability to control pain\u00a0<b>(self-efficacy)<\/b><\/li>\r\n \t<li>prior learning experiences (either direct experience or observational learning)<\/li>\r\n \t<li>expectations of recovery<\/li>\r\n \t<li>current emotional states (<b>which include catastrophizing, anxiety, depression<\/b>)<\/li>\r\n<\/ul>\r\nWhile the above models explain how fear avoidance becomes a\u00a0<b>maladaptive coping strategy<\/b>\u00a0and barrier to recovery in patients with chronic pain, this information may also be relevant to clinicians dealing with patients in the acute stages of injury. Any early fear-avoidance behaviours, especially for those who have strong negative emotions about their pain like anxiety or depression<sup id=\"cite_ref-Boersma_and_Linton_12-1\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-Boersma_and_Linton-12\">[12]<\/a><\/sup><sup id=\"cite_ref-Pincus_et_al_22-3\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-Pincus_et_al-22\">[22]<\/a><\/sup>, could be identified. These patients could be monitored so they do not continue this coping strategy once the acute stage has passed. Knowing what factors affect fear-avoidance would help clinicians elicit these behaviours in their patients, expose any wrongful beliefs, attitudes or coping strategies (active vs passive) patient may have about their pain and recovery, and determine the best way to challenge them.\r\n\r\nPhysiotherapists managing these patients may find it useful to categorise the type of fear-avoidance patients exhibit.\u00a0<b>Rainville et a<\/b>l<sup id=\"cite_ref-Rainville_et_al_5-3\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-Rainville_et_al-5\">[5]<\/a><\/sup>\u00a0proposed the following categories:\r\n<ul>\r\n \t<li><b>Misinformed avoiders:<\/b>\u00a0have misinformed beliefs about movement and risk of damage or re-injury; may benefit from accurate information about pain and movement (pain physiology)<\/li>\r\n \t<li><b>Learned avoiders:<\/b>\u00a0learned the behaviour without being aware nor overly distressed about it; may benefit from\u00a0<a title=\"Exercise and Activity in Pain Management\" href=\"https:\/\/physio-pedia.com\/Exercise_and_Activity_in_Pain_Management\">gradual exposure to activity depending<\/a>\u00a0on pain tolerance<\/li>\r\n \t<li><b>Affective avoiders:<\/b>\u00a0are distressed and have strong negative cognitions about the pain; may need more\u00a0<a class=\"mw-redirect\" title=\"Psychological basis of pain\" href=\"https:\/\/physio-pedia.com\/Psychological_basis_of_pain\">cognition-based management<\/a>\u00a0and challenge beliefs<\/li>\r\n<\/ul>\r\nWhile not every patient with chronic pain will fall into any specific model or category, presenting an appropriate model may help the patient to realise the cycle of pain and fear-avoidance behaviour that individual has adopted. Unfortunately much of this literature does not solely focus on anxiety alone.<\/li>\r\n \t<li>The following have been commonly reported anxiety symptoms reported in patients with chronic pain: \u201cnot being able to stop worrying,\u201d \u201cworrying about too many different things\u201d and \u201cfeeling as though something awful might happen.\u201d<\/li>\r\n \t<li>A study conducted by Asmundson and Taylor in 1996 suggested the relation between anxiety sensitivity and developing pain-related fear and escape\/avoidance in people with chronic pain<sup id=\"cite_ref-41\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-41\">[41]<\/a><\/sup>.<\/li>\r\n<\/ul>\r\nA study<sup id=\"cite_ref-:1_32-1\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-:1-32\">[32]<\/a><\/sup>\u00a0which aimed to evaluate the prediction of chronic pain looked at anxiety, depression and social stressors as risk factors and the severity of pain at 3, 6 and 12 months in 250 patients. They found a strong correlation between baseline anxiety which predicted pain severity at 12 months. Interestingly no correlation was found for social stressors or depression. In a TMsI study, anxiety was found to predict pain intensity at different time-frames post injury whereas depression, social support, length of hospital stay and self-efficacy had no substantial effect. They summarized that pain predicted anxiety and depression in the first year, but anxiety only predicted pain intensity from 12-24 months. Anxiety symptoms were therefore hypothesized as the primary causative factor of persistent pain in this cohort study. It is therefore important to address patients anxiety early to prevent it persisting and causing a negative barrier to their recovery.\r\n\r\nOne study examined the validity of a single question to screen for depression and anxiety<sup id=\"cite_ref-42\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-42\">[42]<\/a><\/sup>.\r\n<ul>\r\n \t<li>Single question screening tools were compared with validated questionnaires such as mini-international neuropsychiatric interview, Hospital Anxiety and Depression Scale (HADS) and Hopkins symptom checklist.<\/li>\r\n \t<li>It was found that the single questions demonstrated fair ability to detecting anxiety with a sensitivity of 68% and a good ability to exclude anxiety with a specificity of 85%.<\/li>\r\n \t<li>Therefore, single question screening tools are fairly effective in identifying anxiety and could be utilised into early assessments with chronic pain patients.<\/li>\r\n<\/ul>\r\n<h3><span id=\"Depression\" class=\"mw-headline\"><b>Depression<\/b><\/span><\/h3>\r\nDepression is described as a general term for mental disorders which include: sadness, loss of interest or pleasure, feelings of low self-worth and low mood.\r\n<ul>\r\n \t<li>Some frequently reported depressive symptoms among chronic pain patients are: \u201cthinking of suicide\/self harm\u201d and \u201cfeeling down, depressed or hopeless\u201d.<\/li>\r\n \t<li>Depression alone is associated with: chronic MSK<sup id=\"cite_ref-43\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-43\">[43]<\/a><\/sup>\u00a0and widespread pain, knee pain<sup id=\"cite_ref-44\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-44\">[44]<\/a><\/sup>, low back pain and chronic pelvic pain\/prostatitis<sup id=\"cite_ref-45\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-45\">[45]<\/a><\/sup>.<\/li>\r\n \t<li>In a study examining the impact of anxiety and depression in phantom limb pain (PLP) patients, mean depression scores were higher, although non-significant, in patients with non-PLP chronic pain syndromes<sup id=\"cite_ref-46\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-46\">[46]<\/a><\/sup>.<\/li>\r\n<\/ul>\r\nA study of low back pain observed higher prevalence of depression symptoms (13.7%) compared to anxiety symptoms (9.5%)<sup id=\"cite_ref-47\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-47\">[47]<\/a><\/sup>.\r\n<ul>\r\n \t<li>Similarly, a cohort of 400 patients with chronic myofascial or neuropathic pain demonstrated higher prevalence of depression (93%) than anxiety (72%)<sup id=\"cite_ref-48\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-48\">[48]<\/a><\/sup>.<\/li>\r\n<\/ul>\r\n<b>Pain, Anxiety and Depression<\/b>\u00a0Anxiety and depression appear to be overlapping conditions. Much of the literature has acknowledged their joint effect in chronic pain patients<sup id=\"cite_ref-49\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-49\">[49]<\/a><\/sup>.\r\n\r\nA study in Brazil, which included 400 chronic pain patients, observed 54% with both of the psychiatric disorders compared to anxiety (18%) or depression (7%) alone<sup id=\"cite_ref-50\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-50\">[50]<\/a><\/sup>. In patients with chronic musculoskeletal pain, those with both anxiety and depression experienced the greatest pain severity, a highly significant finding. Furthermore combined psychiatric co-morbidity was strongly associated with disability days with the following: 18.1 days in pain-only patients, 32.2 in those with pain and anxiety, 38.0 days in those with pain and depression and 42.6 days in those with all three conditions. This study also found a significant correlation with reduced QoL when all three conditions presented together<sup id=\"cite_ref-:2_33-1\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-:2-33\">[33]<\/a><\/sup>.\r\n\r\nThus it comes into question as to why these psychiatric disorders:\r\na) present together,\r\nb) develop with the onset of traumatic pain,\r\nc) appear to cause acute pain to persist; and\r\nd) cause more severe functional deficit together than when presenting independently.\r\n\r\nMany studies have looked into the pathophysiology of pain, anxiety and depression to find answers and to facilitate more successful treatment of these complex conditions<sup id=\"cite_ref-51\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-51\">[51]<\/a><\/sup><sup id=\"cite_ref-52\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-52\">[52]<\/a><\/sup><sup id=\"cite_ref-53\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-53\">[53]<\/a><\/sup>.\r\n<h2><span id=\"Impact_of_Depression_and_Anxiety_on_the_Pain_Experience\" class=\"mw-headline\">Impact of Depression and Anxiety on the Pain Experience<\/span><\/h2>\r\n<h3><span id=\"Anxiety_2\" class=\"mw-headline\">Anxiety<\/span><\/h3>\r\nA physiological model of anxiety may explain its role in pain perception;\r\n<ul>\r\n \t<li>It is known that a state of acute anxiety stimulates the sympathetic nervous system (SNS), causing increased muscle tension, increased nociceptive input and increased sensitivity to pain stimuli, thus explaining why anxiety may correspond to increased pain perception.<\/li>\r\n \t<li>Corticosteroid hormones, for example, released after stress may play a part in pain modulation<sup id=\"cite_ref-:3_54-0\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-:3-54\">[54]<\/a><\/sup>, as cortisol (a steroid hormone known to stimulate the SNS) has been found in higher levels in those with chronic pain<sup id=\"cite_ref-55\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-55\">[55]<\/a><\/sup>.<\/li>\r\n \t<li>Furthermore neurotransmitters, neuropeptides and pro-inflammatory cytokines have been found to either mediate or modulate pain. Therefore, these physiological processes overlap in both anxiety and pain.<\/li>\r\n \t<li>There is evidence accumulating to show atypical sensory processing in the brain and dysfunction of skeletal muscle nociception<sup id=\"cite_ref-:3_54-1\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-:3-54\">[54]<\/a><\/sup><sup id=\"cite_ref-56\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-56\">[56]<\/a><\/sup>, however the latter may not explain pain persistence in non-MSK pathologies where chronic pain has clearly been observed.<\/li>\r\n \t<li>The \u201cpain matrix\u201d<sup id=\"cite_ref-57\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-57\">[57]<\/a><\/sup>, responsible for modulation of pain signals in the central nervous system (CNS), shares elements with brain networks responsible for stress, identifying another connection to the SNS. Pain contributes to feelings of anxiety due to fear of the cause, and if this persists a state of hypervigilance and avoidance behaviors may develop as a \u201cmaladaptive\u201d coping response.<\/li>\r\n \t<li>A more permanent state of anxiety results in chronic muscle tension and anticipatory anxiety which leads to further disability.<\/li>\r\n \t<li>Decreases in activities, especially those which provide meaning and reinforcement, may result in greater social isolation, decreased self-efficacy, increased feelings of uselessness and subsequent increases in anxiety and depression symptoms.<\/li>\r\n<\/ul>\r\nThis theory is closely interlinked with the \u2018fear-avoidance\u2019 model, a cycle prospected to play a vital part in sustenance of pain behaviors and which has been shown to be an independent predictor of pain severity and disability among those with MSK pain<sup id=\"cite_ref-58\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-58\">[58]<\/a><\/sup>.\r\n\r\nFunctional magnetic resonance imaging (fMRI) studies along with blood-oxygenated level dependent contrast (BOLD) techniques have been utilized in various studies to map common areas of brain activation by detecting changes in blood flow. These studies have been able to map similar areas of brain activation for anxiety and pain. It has been shown that there is exaggerated brain response in patients with social anxiety disorder<sup id=\"cite_ref-:0_59-0\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-:0-59\">[59]<\/a><\/sup>.\r\n<ul>\r\n \t<li>In one study\u00a0these methods were used to identify highlighted areas in the brain when anxious patients reacted to negative self-beliefs.<\/li>\r\n \t<li>The areas identified were the midline cortical regions such as: the ventromedial pre-frontal cortex (PFC), dorsomedial PFC, posterior cingulate cortex (implicated in self-referential process), emotional center (amygdala) and memory area (hippocampal gyrus)<sup id=\"cite_ref-:0_59-1\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-:0-59\">[59]<\/a><\/sup>.<\/li>\r\n \t<li>This indicates that there are integrated neural pathways associated with anxiety, pain processing, memory and concept of the self \u2013 the latter potentially implicating personality traits into anxiety and pain.<\/li>\r\n<\/ul>\r\n<h3><span id=\"Depression_2\" class=\"mw-headline\">Depression<\/span><\/h3>\r\nThere is less known evidence about the physiology of depression compared to anxiety, however it has been postulated that people with depression or a depressive personality have a greater sensitivity to acute and chronic pain<sup id=\"cite_ref-:2_33-2\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-:2-33\">[33]<\/a><\/sup>.\r\n<ul>\r\n \t<li>Similarly to anxiety, excessive sympathetic activity and elevated pro-inflammatory cytokine production is also postulated in the etiology of depression, thus providing a potential physiological link between all three conditions and a possible reason for the overlapping prevalence within patient presentations.<\/li>\r\n \t<li>fMRI studies have been conducted in patients with depressive disorders and similar findings have been identified as in anxious patients.<\/li>\r\n \t<li>In diabetic patients with depression, similar brain activation was identified in both neuropathic and non-neuropathic pain patients; these were PFC, thalamus, insula and anterior cingulate cortex.<\/li>\r\n \t<li>Despite these CNS observations, it is known that in healthy, non-depressed human volunteers, pain-intensity related hemodynamic changes have been observed in all of the above brain sites<sup id=\"cite_ref-60\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-60\">[60]<\/a><\/sup>. Further studies compared the response of brain areas with BOLD techniques:\r\n<ul>\r\n \t<li>13 patients suffering from an acute episode of major depressive disorder (MDD) were investigated during painful stimulus application and compared to 13 control subjects<sup id=\"cite_ref-61\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-61\">[61]<\/a><\/sup>.<\/li>\r\n \t<li>The results demonstrated increased activation of the pain matrix and increased thermal pain thresholds compared to healthy subjects.<\/li>\r\n \t<li>They speculated that the brain area activation may be linked to an underlying prefrontal psychopathology in depression.<\/li>\r\n<\/ul>\r\n<\/li>\r\n<\/ul>\r\nIn another BOLD study<sup id=\"cite_ref-62\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-62\">[62]<\/a><\/sup>\u00a0patients with diagnosed MDD showed greater activation of pain-related brain sites (right insular, dorsal anterior cingulate, right amygdala) during anticipation of painful, heat stimuli compared with healthy subjects.\r\n<ul>\r\n \t<li>Furthermore, in MDD subjects, greater activation of the amygdala was associated with greater levels of perceived helplessness.<\/li>\r\n \t<li>This implies anticipation of pain may further pain experience and activation of the pain matrix potentially contributing to persistence of pain.<\/li>\r\n<\/ul>\r\nIn other studies, the cingulate and PFC have also been implicated in pain modulation and may contribute to chronic pain associated with fibromyalgia syndrome<sup id=\"cite_ref-:3_54-2\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-:3-54\">[54]<\/a><\/sup>. The brain is perceived as a mediator for nociceptive pain, but also for pain behaviors which are known to influence pain itself, likely causing its persistence. \u2018Central sensitization syndrome\u2019 (CSS) is a highly recognized symptom of chronic pain syndrome. Hyperesthesia and allodynia (increased sensitivity to painful and non-painful stimuli) are features of CSS and have been suggested to be mediated by synaptic strengthening and neuroplastic change at multiple CNS levels,\u00a0further strengthening the hypothesis of the brain's role in nociceptive changes in the periphery<sup id=\"cite_ref-63\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-63\">[63]<\/a><\/sup>.\r\n<ul>\r\n \t<li>Other researchers have proposed that anxiety and depression may have similar physiological effects on programming in the brain providing more evidence for the relationship<sup id=\"cite_ref-:2_33-3\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-:2-33\">[33]<\/a><\/sup>. It is clear from research studies that physiology of depression, anxiety and pain are overlapping. However, it is difficult to differentiate the primary versus secondary cause.<span id=\"Catastrophising\"><\/span><\/li>\r\n<\/ul>\r\n<h2><span id=\"Catastrophising\" class=\"mw-headline\">Catastrophizing<\/span><\/h2>\r\nPain catastrophizing refers to a negative cognitive-affective response to anticipated or actual pain<sup id=\"cite_ref-64\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-64\">[64]<\/a><\/sup>. It was formally introduced by Albert Ellis and was used to describe a maladaptive cognitive style in those with anxiety and depressive disorders<sup id=\"cite_ref-65\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-65\">[65]<\/a><\/sup>. The research focused on the fact that catastrophizing was an exaggerated and negative cognitive and emotional response during an actual or anticipated painful stimulation. Catastrophizing is often characterized by people magnifying their feelings about painful situations and ruminating about them, which can combine with feelings of helplessness<sup id=\"cite_ref-66\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-66\">[66]<\/a><\/sup>. Catastrophizing plays an important role in models of pain chronicity, showing a high correlation with both pain intensity and disability<sup id=\"cite_ref-67\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-67\">[67]<\/a><\/sup>. However, it is also apparent that fear-avoidance and depression are important predictors of pain intensity and disability<sup id=\"cite_ref-68\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-68\">[68]<\/a><\/sup>. High correlations between fear-avoidance and pain catastrophizing have been found<sup id=\"cite_ref-69\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-69\">[69]<\/a><\/sup>. In studies, however, only pain catastrophizing predicted pain intensity<sup id=\"cite_ref-70\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-70\">[70]<\/a><\/sup>. Additionally, catastrophizing has also been linked to adverse pain outcomes<sup id=\"cite_ref-71\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-71\">[71]<\/a><\/sup><sup id=\"cite_ref-72\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-72\">[72]<\/a><\/sup><sup id=\"cite_ref-73\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-73\">[73]<\/a><\/sup>. It can therefore be surmised that a reduction in pain catastrophizing will lead to a reduction in pain and disability<sup id=\"cite_ref-74\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-74\">[74]<\/a><\/sup>. This highlights the importance of a multifactorial approach to pain management and the significance catastrophizing has in molding the pain experience.\r\n<h3><span id=\"How_Can_We_Assess_the_Levels_of_Catastrophizing.3F\" class=\"mw-headline\">How Can We Assess the Levels of Catastrophizing?<\/span><\/h3>\r\nResearch has developed self-report instruments that can be used in various populations<sup id=\"cite_ref-75\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-75\">[75]<\/a><\/sup>. Most commonly used is a 3-factor hierarchical structure for pain catastrophizing called\u00a0<a title=\"Pain Catastrophizing Scale\" href=\"https:\/\/physio-pedia.com\/Pain_Catastrophizing_Scale\">The Pain Catastrophizing Scale<\/a>\u00a0(PCS). This scale incorporates magnification, rumination and helplessness. Highlighting the need for a multifactorial approach, participants are asked to rate the extent to which they experience each item by recalling previous experiences with pain<sup id=\"cite_ref-76\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-76\">[76]<\/a><\/sup>. More negative experiences with pain can correlate with higher levels of pain intensity and disability<sup id=\"cite_ref-77\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-77\">[77]<\/a><\/sup>.\r\n\r\n<b>Why does this happen?<\/b>\r\n\r\nThere are various theoretical mechanisms of action including the appraisal theory<sup id=\"cite_ref-78\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-78\">[78]<\/a><\/sup>, where the levels of helplessness the person is feeling will affect their ability to cope. Additional mechanisms include attention bias\/information processing<sup id=\"cite_ref-79\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-79\">[79]<\/a><\/sup>, where the catastrophizing amplifies the experience of pain via amplified attention biases to sensory and affective pain information<sup id=\"cite_ref-80\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-80\">[80]<\/a><\/sup>, and an inability to suppress pain-related cognitions<sup id=\"cite_ref-81\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-81\">[81]<\/a><\/sup>. Evidence has also suggested that there is an increased nociceptive transmission via spinal gating mechanisms and a central sensitization of pain. This may represent a central nervous mechanism which is contributing to the development, maintenance and aggravation of persistent pain<sup id=\"cite_ref-82\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-82\">[82]<\/a><\/sup><sup id=\"cite_ref-83\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-83\">[83]<\/a><\/sup><sup id=\"cite_ref-84\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-84\">[84]<\/a><\/sup>.\r\n\r\nIt is therefore apparent that both psychological and physiological factors play a major role in the perception, maintenance, experience and management of pain; and both directly influence each other. It is important to recognize these factors in both the acute and chronic pain patient in order to understand what aspects of their pain are a barrier to recovery.\r\n<div class=\"thumb tright thumbnail pull-right\">\r\n<div class=\"thumbinner\"><a class=\"image\" href=\"https:\/\/physio-pedia.com\/File:Figure_1_The_Stress_Response.png\"><img class=\"thumbimage img-responsive alignright\" src=\"https:\/\/physio-pedia.com\/images\/thumb\/c\/c4\/Figure_1_The_Stress_Response.png\/300px-Figure_1_The_Stress_Response.png\" alt=\"Figure 1 The Stress Response.png\" width=\"279\" height=\"24\" \/><\/a>\r\n<div class=\"thumbcaption caption\">\r\n<div class=\"magnify\"><span style=\"color: #077fab;font-size: 1.15em;font-weight: 600\">Chronic Stress and Pain: Physiological Effects<\/span><\/div>\r\n<\/div>\r\n<\/div>\r\n<\/div>\r\nAs discussed previously, psycho-social factors including depression, anxiety or fear-avoidance all factor into the pain experience and have distinct\u00a0<a title=\"Chronic Pain and the Brain\" href=\"https:\/\/physio-pedia.com\/Chronic_Pain_and_the_Brain\">acute and chronic effects on the brain<\/a>. Additionally, chronic stress contributes to pain sensitization via physiological changes occurring during the sympathetic response<sup id=\"cite_ref-:4_85-0\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-:4-85\">[85]<\/a><\/sup>. These changes are mediated primarily by cortisol, an anti-inflammatory neuroendocrine hormone. In the short term, the stress response is a physiologic flight-or-fight response, in which secretion of epinephrine and norepinephrine (sympathetic neurotransmitters) and cortisol promote survival. The immediate pro-inflammatory stage of the stress response, governed by the sympathetic neurotransmitters, is marked by increased heart rate, blood pressure, and respiration, as well as vasoconstriction and pupillary dilation. The latter stage of the stress response occurs as a delayed, systematic increase in cortisol levels 15 minutes after the initial stress response. The downstream effect of CRH subsequently triggers the release of adrenocorticotropic hormone (ACTH) via the anterior pituitary gland, which signals the release of cortisol from the adrenal cortex. This pathway, from hypothalamus to adrenal cortex, via CRH and ACTH signaling the release of cortisol, is referred to as the\u00a0<\/span><b style=\"font-size: 1rem;text-align: initial\">Hypothalamic-Pituitary-Adrenal (HPA) Axis<\/b><sup id=\"cite_ref-86\" class=\"reference\" style=\"text-align: initial\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-86\">[86]<\/a><\/sup><span style=\"font-size: 1rem;text-align: initial\">. Although short-term activation of the HPA axis and cortisol release is anti-inflammatory and adaptive, prolonged and repeated stress exposure may lead to HPA axis exhaustion and cortisol dysfunction, including depletion, downregulation, and impairment of the negative feedback system<\/span><sup id=\"cite_ref-:4_85-2\" class=\"reference\" style=\"text-align: initial\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-:4-85\">[85]<\/a><\/sup><span style=\"font-size: 1rem;text-align: initial\">. Catastrophizing and other maladaptive pain beliefs may similarly lead to reactivation of the stress response, prolonged cortisol secretion and dysfunction. These changes ultimately result in reduced cortisol efficacy in the secondary, anti-inflammatory, stage of the stress response.<\/span><\/div>\r\n<\/div>\r\n<\/div>\r\n<\/div>\r\n<h3><span id=\"Cortisol_Dysfunction:_Effects_on_Pain\" class=\"mw-headline\">Cortisol Dysfunction: Effects on Pain<\/span><\/h3>\r\nCortisol dysfunction has been implicated in various inflammatory and autoimmune diseases, ranging from osteoporosis, rheumatoid arthritis, and chronic fatigue syndrome, among others<sup id=\"cite_ref-:4_85-3\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-:4-85\">[85]<\/a><\/sup>. HPA axis exhaustion and cortisol dysfunction can manifest as stress-induced hypocortisolism and are associated with musculoskeletal pain conditions including chronic low back pain and fibromyalgia. Additional studies have suggested hypocortisolism as a predictive risk factor for musculoskeletal pain<sup id=\"cite_ref-87\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-87\">[87]<\/a><\/sup>.\r\n\r\nTherefore, it is critical to address both pain and non-pain-related stress in treating pain. Such measures may include objective cortisol readings in a medical setting, or subjective self-reports of stress as part of a screening or physiotherapy evaluation. By identifying stress factors or maladaptive coping strategies early in the screening process,\u00a0<a title=\"Chronic Pain\" href=\"https:\/\/physio-pedia.com\/Chronic_Pain\">providers can educate patients about chronic pain<\/a>\u00a0and the interplay between psychosocial factors and pain. Physiotherapists may also utilize\u00a0<a title=\"Pain Neuroscience Education (PNE)\" href=\"https:\/\/physio-pedia.com\/Pain_Neuroscience_Education_(PNE)\">pain neuroscience education<\/a>. Knowing what factors affect fear-avoidance would help clinicians elicit these behaviours in their patients, expose any wrongful beliefs, attitudes or coping strategies (active vs passive) patient may have about their pain and recovery, and determine the best way to challenge them.<\/p>\n<p>Physiotherapists managing these patients may find it useful to categorise the type of fear-avoidance patients exhibit.\u00a0<b>Rainville et a<\/b>l<sup id=\"cite_ref-Rainville_et_al_5-3\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-Rainville_et_al-5\">[5]<\/a><\/sup>\u00a0proposed the following categories:<\/p>\n<ul>\n<li><b>Misinformed avoiders:<\/b>\u00a0have misinformed beliefs about movement and risk of damage or re-injury; may benefit from accurate information about pain and movement (pain physiology)<\/li>\n<li><b>Learned avoiders:<\/b>\u00a0learned the behaviour without being aware nor overly distressed about it; may benefit from\u00a0<a title=\"Exercise and Activity in Pain Management\" href=\"https:\/\/physio-pedia.com\/Exercise_and_Activity_in_Pain_Management\">gradual exposure to activity depending<\/a>\u00a0on pain tolerance<\/li>\n<li><b>Affective avoiders:<\/b>\u00a0are distressed and have strong negative cognitions about the pain; may need more\u00a0<a class=\"mw-redirect\" title=\"Psychological basis of pain\" href=\"https:\/\/physio-pedia.com\/Psychological_basis_of_pain\">cognition-based management<\/a>\u00a0and challenge beliefs<\/li>\n<\/ul>\n<p>While not every patient with chronic pain will fall into any specific model or category, presenting an appropriate model may help the patient to realise the cycle of pain and fear-avoidance behaviour that individual has adopted. Furthermore, explaining how fear avoidance affects the physiological feelings they may be experiencing will help them to understand how their perception of pain may be altered<sup id=\"cite_ref-Turk_and_Okifugi_4-4\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-Turk_and_Okifugi-4\">[4]<\/a><\/sup><sup id=\"cite_ref-Vlaeyen_and_Linton_7-7\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-Vlaeyen_and_Linton-7\">[7]<\/a><\/sup><sup id=\"cite_ref-Leeuw_et_al_6-2\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-Leeuw_et_al-6\">[6]<\/a><\/sup>. For example:<\/p>\n<ul>\n<li>Increased physiological arousal\/heightened muscle reactivity<\/li>\n<li>Associated anxiety and hyper-vigilance leads to increased attention to expected painful stimulus which in itself can intensify the pain<\/li>\n<li>Reduced participation in valued activities like work, leisure and social contacts can lead to mood changes like irritability, frustration and depression; these mood changes could further affect perception of pain<\/li>\n<li>Disuse or disability from persistent avoidance behavior will lead to pain with much less provocation than before (lower the threshold for experiencing future pain).<\/li>\n<\/ul>\n<p>Several\u00a0<b>self-reported measures<\/b>\u00a0of fear avoidance could be used in the clinic to help assess fear avoidance. Unfortunately much of this literature does not solely focus on anxiety alone.<\/li>\n<li>The following have been commonly reported anxiety symptoms reported in patients with chronic pain: \u201cnot being able to stop worrying,\u201d \u201cworrying about too many different things\u201d and \u201cfeeling as though something awful might happen.\u201d<\/li>\n<li>A study conducted by Asmundson and Taylor in 1996 suggested the relation between anxiety sensitivity and developing pain-related fear and escape\/avoidance in people with chronic pain<sup id=\"cite_ref-41\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-41\">[41]<\/a><\/sup>.<\/li>\n<\/ul>\n<p>A study<sup id=\"cite_ref-:1_32-1\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-:1-32\">[32]<\/a><\/sup>\u00a0which aimed to evaluate the prediction of chronic pain looked at anxiety, depression and social stressors as risk factors and the severity of pain at 3, 6 and 12 months in 250 patients. They found a strong correlation between baseline anxiety which predicted pain severity at 12 months. Interestingly no correlation was found for social stressors or depression. In a TMsI study, anxiety was found to predict pain intensity at different time-frames post injury whereas depression, social support, length of hospital stay and self-efficacy had no substantial effect. They summarized that pain predicted anxiety and depression in the first year, but anxiety only predicted pain intensity from 12-24 months. Anxiety symptoms were therefore hypothesized as the primary causative factor of persistent pain in this cohort study. It is therefore important to address patients anxiety early to prevent it persisting and causing a negative barrier to their recovery.<\/p>\n<p>One study examined the validity of a single question to screen for depression and anxiety<sup id=\"cite_ref-42\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-42\">[42]<\/a><\/sup>.<\/p>\n<ul>\n<li>Single question screening tools were compared with validated questionnaires such as mini-international neuropsychiatric interview, Hospital Anxiety and Depression Scale (HADS) and Hopkins symptom checklist.<\/li>\n<li>It was found that the single questions demonstrated fair ability to detecting anxiety with a sensitivity of 68% and a good ability to exclude anxiety with a specificity of 85%.<\/li>\n<li>Therefore, single question screening tools are fairly effective in identifying anxiety and could be utilised into early assessments with chronic pain patients.<\/li>\n<\/ul>\n<h3><span id=\"Depression\" class=\"mw-headline\"><b>Depression<\/b><\/span><\/h3>\n<p>Depression is described as a general term for mental disorders which include: sadness, loss of interest or pleasure, feelings of low self-worth and low mood.<\/p>\n<ul>\n<li>Some frequently reported depressive symptoms among chronic pain patients are: \u201cthinking of suicide\/self harm\u201d and \u201cfeeling down, depressed or hopeless\u201d.<\/li>\n<li>Depression alone is associated with: chronic MSK<sup id=\"cite_ref-43\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-43\">[43]<\/a><\/sup>\u00a0and widespread pain, knee pain<sup id=\"cite_ref-44\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-44\">[44]<\/a><\/sup>, low back pain and chronic pelvic pain\/prostatitis<sup id=\"cite_ref-45\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-45\">[45]<\/a><\/sup>.<\/li>\n<li>In a study examining the impact of anxiety and depression in phantom limb pain (PLP) patients, mean depression scores were higher, although non-significant, in patients with non-PLP chronic pain syndromes<sup id=\"cite_ref-46\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-46\">[46]<\/a><\/sup>.<\/li>\n<\/ul>\n<p>A study of low back pain observed higher prevalence of depression symptoms (13.7%) compared to anxiety symptoms (9.5%)<sup id=\"cite_ref-47\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-47\">[47]<\/a><\/sup>.<\/p>\n<ul>\n<li>Similarly, a cohort of 400 patients with chronic myofascial or neuropathic pain demonstrated higher prevalence of depression (93%) than anxiety (72%)<sup id=\"cite_ref-48\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-48\">[48]<\/a><\/sup>.<\/li>\n<\/ul>\n<p><b>Pain, Anxiety and Depression<\/b>\u00a0Anxiety and depression appear to be overlapping conditions. Much of the literature has acknowledged their joint effect in chronic pain patients<sup id=\"cite_ref-49\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-49\">[49]<\/a><\/sup>.<\/p>\n<p>A study in Brazil, which included 400 chronic pain patients, observed 54% with both of the psychiatric disorders compared to anxiety (18%) or depression (7%) alone<sup id=\"cite_ref-50\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-50\">[50]<\/a><\/sup>. In patients with chronic musculoskeletal pain, those with both anxiety and depression experienced the greatest pain severity, a highly significant finding. Furthermore combined psychiatric co-morbidity was strongly associated with disability days with the following: 18.1 days in pain-only patients, 32.2 in those with pain and anxiety, 38.0 days in those with pain and depression and 42.6 days in those with all three conditions. This study also found a significant correlation with reduced QoL when all three conditions presented together<sup id=\"cite_ref-:2_33-1\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-:2-33\">[33]<\/a><\/sup>.<\/p>\n<p>Thus it comes into question as to why these psychiatric disorders:<br \/>\na) present together,<br \/>\nb) develop with the onset of traumatic pain,<br \/>\nc) appear to cause acute pain to persist; and<br \/>\nd) cause more severe functional deficit together than when presenting independently.<\/p>\n<p>Many studies have looked into the pathophysiology of pain, anxiety and depression to find answers and to facilitate more successful treatment of these complex conditions<sup id=\"cite_ref-51\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-51\">[51]<\/a><\/sup><sup id=\"cite_ref-52\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-52\">[52]<\/a><\/sup><sup id=\"cite_ref-53\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-53\">[53]<\/a><\/sup>.<\/p>\n<h2><span id=\"Impact_of_Depression_and_Anxiety_on_the_Pain_Experience\" class=\"mw-headline\">Impact of Depression and Anxiety on the Pain Experience<\/span><\/h2>\n<h3><span id=\"Anxiety_2\" class=\"mw-headline\">Anxiety<\/span><\/h3>\n<p>A physiological model of anxiety may explain its role in pain perception;<\/p>\n<ul>\n<li>It is known that a state of acute anxiety stimulates the sympathetic nervous system (SNS), causing increased muscle tension, increased nociceptive input and increased sensitivity to pain stimuli, thus explaining why anxiety may correspond to increased pain perception.<\/li>\n<li>Corticosteroid hormones, for example, released after stress may play a part in pain modulation<sup id=\"cite_ref-:3_54-0\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-:3-54\">[54]<\/a><\/sup>, as cortisol (a steroid hormone known to stimulate the SNS) has been found in higher levels in those with chronic pain<sup id=\"cite_ref-55\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-55\">[55]<\/a><\/sup>.<\/li>\n<li>Furthermore neurotransmitters, neuropeptides and pro-inflammatory cytokines have been found to either mediate or modulate pain. Therefore, these physiological processes overlap in both anxiety and pain.<\/li>\n<li>There is evidence accumulating to show atypical sensory processing in the brain and dysfunction of skeletal muscle nociception<sup id=\"cite_ref-:3_54-1\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-:3-54\">[54]<\/a><\/sup><sup id=\"cite_ref-56\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-56\">[56]<\/a><\/sup>, however the latter may not explain pain persistence in non-MSK pathologies where chronic pain has clearly been observed.<\/li>\n<li>The \u201cpain matrix\u201d<sup id=\"cite_ref-57\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-57\">[57]<\/a><\/sup>, responsible for modulation of pain signals in the central nervous system (CNS), shares elements with brain networks responsible for stress, identifying another connection to the SNS. Pain contributes to feelings of anxiety due to fear of the cause, and if this persists a state of hypervigilance and avoidance behaviors may develop as a \u201cmaladaptive\u201d coping response.<\/li>\n<li>A more permanent state of anxiety results in chronic muscle tension and anticipatory anxiety which leads to further disability.<\/li>\n<li>Decreases in activities, especially those which provide meaning and reinforcement, may result in greater social isolation, decreased self-efficacy, increased feelings of uselessness and subsequent increases in anxiety and depression symptoms.<\/li>\n<\/ul>\n<p>This theory is closely interlinked with the \u2018fear-avoidance\u2019 model, a cycle prospected to play a vital part in sustenance of pain behaviors and which has been shown to be an independent predictor of pain severity and disability among those with MSK pain<sup id=\"cite_ref-58\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-58\">[58]<\/a><\/sup>.<\/p>\n<p>Functional magnetic resonance imaging (fMRI) studies along with blood-oxygenated level dependent contrast (BOLD) techniques have been utilized in various studies to map common areas of brain activation by detecting changes in blood flow. These studies have been able to map similar areas of brain activation for anxiety and pain. It has been shown that there is exaggerated brain response in patients with social anxiety disorder<sup id=\"cite_ref-:0_59-0\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-:0-59\">[59]<\/a><\/sup>.<\/p>\n<ul>\n<li>In one study\u00a0these methods were used to identify highlighted areas in the brain when anxious patients reacted to negative self-beliefs.<\/li>\n<li>The areas identified were the midline cortical regions such as: the ventromedial pre-frontal cortex (PFC), dorsomedial PFC, posterior cingulate cortex (implicated in self-referential process), emotional center (amygdala) and memory area (hippocampal gyrus)<sup id=\"cite_ref-:0_59-1\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-:0-59\">[59]<\/a><\/sup>.<\/li>\n<li>This indicates that there are integrated neural pathways associated with anxiety, pain processing, memory and concept of the self \u2013 the latter potentially implicating personality traits into anxiety and pain.<\/li>\n<\/ul>\n<h3><span id=\"Depression_2\" class=\"mw-headline\">Depression<\/span><\/h3>\n<p>There is less known evidence about the physiology of depression compared to anxiety, however it has been postulated that people with depression or a depressive personality have a greater sensitivity to acute and chronic pain<sup id=\"cite_ref-:2_33-2\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-:2-33\">[33]<\/a><\/sup>.<\/p>\n<ul>\n<li>Similarly to anxiety, excessive sympathetic activity and elevated pro-inflammatory cytokine production is also postulated in the etiology of depression, thus providing a potential physiological link between all three conditions and a possible reason for the overlapping prevalence within patient presentations.<\/li>\n<li>fMRI studies have been conducted in patients with depressive disorders and similar findings have been identified as in anxious patients.<\/li>\n<li>In diabetic patients with depression, similar brain activation was identified in both neuropathic and non-neuropathic pain patients; these were PFC, thalamus, insula and anterior cingulate cortex.<\/li>\n<li>Despite these CNS observations, it is known that in healthy, non-depressed human volunteers, pain-intensity related hemodynamic changes have been observed in all of the above brain sites<sup id=\"cite_ref-60\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-60\">[60]<\/a><\/sup>. Further studies compared the response of brain areas with BOLD techniques:\n<ul>\n<li>13 patients suffering from an acute episode of major depressive disorder (MDD) were investigated during painful stimulus application and compared to 13 control subjects<sup id=\"cite_ref-61\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-61\">[61]<\/a><\/sup>.<\/li>\n<li>The results demonstrated increased activation of the pain matrix and increased thermal pain thresholds compared to healthy subjects.<\/li>\n<li>They speculated that the brain area activation may be linked to an underlying prefrontal psychopathology in depression.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p>In another BOLD study<sup id=\"cite_ref-62\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-62\">[62]<\/a><\/sup>\u00a0patients with diagnosed MDD showed greater activation of pain-related brain sites (right insular, dorsal anterior cingulate, right amygdala) during anticipation of painful, heat stimuli compared with healthy subjects.<\/p>\n<ul>\n<li>Furthermore, in MDD subjects, greater activation of the amygdala was associated with greater levels of perceived helplessness.<\/li>\n<li>This implies anticipation of pain may further pain experience and activation of the pain matrix potentially contributing to persistence of pain.<\/li>\n<\/ul>\n<p>In other studies, the cingulate and PFC have also been implicated in pain modulation and may contribute to chronic pain associated with fibromyalgia syndrome<sup id=\"cite_ref-:3_54-2\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-:3-54\">[54]<\/a><\/sup>. The brain is perceived as a mediator for nociceptive pain, but also for pain behaviors which are known to influence pain itself, likely causing its persistence. \u2018Central sensitization syndrome\u2019 (CSS) is a highly recognized symptom of chronic pain syndrome. Hyperesthesia and allodynia (increased sensitivity to painful and non-painful stimuli) are features of CSS and have been suggested to be mediated by synaptic strengthening and neuroplastic change at multiple CNS levels,\u00a0further strengthening the hypothesis of the brain&#8217;s role in nociceptive changes in the periphery<sup id=\"cite_ref-63\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-63\">[63]<\/a><\/sup>.<\/p>\n<ul>\n<li>Other researchers have proposed that anxiety and depression may have similar physiological effects on programming in the brain providing more evidence for the relationship<sup id=\"cite_ref-:2_33-3\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-:2-33\">[33]<\/a><\/sup>. It is clear from research studies that physiology of depression, anxiety and pain are overlapping. However, it is difficult to differentiate the primary versus secondary cause.<span id=\"Catastrophising\"><\/span><\/li>\n<\/ul>\n<h2><span id=\"Catastrophising\" class=\"mw-headline\">Catastrophizing<\/span><\/h2>\n<p>Pain catastrophizing refers to a negative cognitive-affective response to anticipated or actual pain<sup id=\"cite_ref-64\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-64\">[64]<\/a><\/sup>. It was formally introduced by Albert Ellis and was used to describe a maladaptive cognitive style in those with anxiety and depressive disorders<sup id=\"cite_ref-65\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-65\">[65]<\/a><\/sup>. The research focused on the fact that catastrophizing was an exaggerated and negative cognitive and emotional response during an actual or anticipated painful stimulation. Catastrophizing is often characterized by people magnifying their feelings about painful situations and ruminating about them, which can combine with feelings of helplessness<sup id=\"cite_ref-66\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-66\">[66]<\/a><\/sup>. Catastrophizing plays an important role in models of pain chronicity, showing a high correlation with both pain intensity and disability<sup id=\"cite_ref-67\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-67\">[67]<\/a><\/sup>. However, it is also apparent that fear-avoidance and depression are important predictors of pain intensity and disability<sup id=\"cite_ref-68\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-68\">[68]<\/a><\/sup>. High correlations between fear-avoidance and pain catastrophizing have been found<sup id=\"cite_ref-69\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-69\">[69]<\/a><\/sup>. In studies, however, only pain catastrophizing predicted pain intensity<sup id=\"cite_ref-70\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-70\">[70]<\/a><\/sup>. Additionally, catastrophizing has also been linked to adverse pain outcomes<sup id=\"cite_ref-71\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-71\">[71]<\/a><\/sup><sup id=\"cite_ref-72\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-72\">[72]<\/a><\/sup><sup id=\"cite_ref-73\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-73\">[73]<\/a><\/sup>. It can therefore be surmised that a reduction in pain catastrophizing will lead to a reduction in pain and disability<sup id=\"cite_ref-74\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-74\">[74]<\/a><\/sup>. This highlights the importance of a multifactorial approach to pain management and the significance catastrophizing has in molding the pain experience.<\/p>\n<h3><span id=\"How_Can_We_Assess_the_Levels_of_Catastrophizing.3F\" class=\"mw-headline\">How Can We Assess the Levels of Catastrophizing?<\/span><\/h3>\n<p>Research has developed self-report instruments that can be used in various populations<sup id=\"cite_ref-75\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-75\">[75]<\/a><\/sup>. Most commonly used is a 3-factor hierarchical structure for pain catastrophizing called\u00a0<a title=\"Pain Catastrophizing Scale\" href=\"https:\/\/physio-pedia.com\/Pain_Catastrophizing_Scale\">The Pain Catastrophizing Scale<\/a>\u00a0(PCS). This scale incorporates magnification, rumination and helplessness. Highlighting the need for a multifactorial approach, participants are asked to rate the extent to which they experience each item by recalling previous experiences with pain<sup id=\"cite_ref-76\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-76\">[76]<\/a><\/sup>. More negative experiences with pain can correlate with higher levels of pain intensity and disability<sup id=\"cite_ref-77\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-77\">[77]<\/a><\/sup>.<\/p>\n<p><b>Why does this happen?<\/b><\/p>\n<p>There are various theoretical mechanisms of action including the appraisal theory<sup id=\"cite_ref-78\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-78\">[78]<\/a><\/sup>, where the levels of helplessness the person is feeling will affect their ability to cope. Additional mechanisms include attention bias\/information processing<sup id=\"cite_ref-79\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-79\">[79]<\/a><\/sup>, where the catastrophizing amplifies the experience of pain via amplified attention biases to sensory and affective pain information<sup id=\"cite_ref-80\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-80\">[80]<\/a><\/sup>, and an inability to suppress pain-related cognitions<sup id=\"cite_ref-81\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-81\">[81]<\/a><\/sup>. Evidence has also suggested that there is an increased nociceptive transmission via spinal gating mechanisms and a central sensitization of pain. This may represent a central nervous mechanism which is contributing to the development, maintenance and aggravation of persistent pain<sup id=\"cite_ref-82\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-82\">[82]<\/a><\/sup><sup id=\"cite_ref-83\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-83\">[83]<\/a><\/sup><sup id=\"cite_ref-84\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-84\">[84]<\/a><\/sup>.<\/p>\n<p>It is therefore apparent that both psychological and physiological factors play a major role in the perception, maintenance, experience and management of pain; and both directly influence each other. It is important to recognize these factors in both the acute and chronic pain patient in order to understand what aspects of their pain are a barrier to recovery.<\/p>\n<div class=\"thumb tright thumbnail pull-right\">\n<div class=\"thumbinner\"><a class=\"image\" href=\"https:\/\/physio-pedia.com\/File:Figure_1_The_Stress_Response.png\"><img loading=\"lazy\" decoding=\"async\" class=\"thumbimage img-responsive alignright\" src=\"https:\/\/physio-pedia.com\/images\/thumb\/c\/c4\/Figure_1_The_Stress_Response.png\/300px-Figure_1_The_Stress_Response.png\" alt=\"Figure 1 The Stress Response.png\" width=\"279\" height=\"24\" \/><\/a><\/p>\n<div class=\"thumbcaption caption\">\n<div class=\"magnify\"><span style=\"color: #077fab;font-size: 1.15em;font-weight: 600\">Chronic Stress and Pain: Physiological Effects<\/span><\/div>\n<\/div>\n<\/div>\n<\/div>\n<p>As discussed previously, psycho-social factors including depression, anxiety or fear-avoidance all factor into the pain experience and have distinct\u00a0<a title=\"Chronic Pain and the Brain\" href=\"https:\/\/physio-pedia.com\/Chronic_Pain_and_the_Brain\">acute and chronic effects on the brain<\/a>. Additionally, chronic stress contributes to pain sensitization via physiological changes occurring during the sympathetic response<sup id=\"cite_ref-:4_85-0\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-:4-85\">[85]<\/a><\/sup>. These changes are mediated primarily by cortisol, an anti-inflammatory neuroendocrine hormone. In the short term, the stress response is a physiologic flight-or-fight response, in which secretion of epinephrine and norepinephrine (sympathetic neurotransmitters) and cortisol promote survival. The immediate pro-inflammatory stage of the stress response, governed by the sympathetic neurotransmitters, is marked by increased heart rate, blood pressure, and respiration, as well as vasoconstriction and pupillary dilation. The latter stage of the stress response occurs as a delayed, systematic increase in cortisol levels 15 minutes after the initial stress response. The downstream effect of CRH subsequently triggers the release of adrenocorticotropic hormone (ACTH) via the anterior pituitary gland, which signals the release of cortisol from the adrenal cortex. This pathway, from hypothalamus to adrenal cortex, via CRH and ACTH signaling the release of cortisol, is referred to as the\u00a0<\/span><b style=\"font-size: 1rem;text-align: initial\">Hypothalamic-Pituitary-Adrenal (HPA) Axis<\/b><sup id=\"cite_ref-86\" class=\"reference\" style=\"text-align: initial\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-86\">[86]<\/a><\/sup><span style=\"font-size: 1rem;text-align: initial\">. Although short-term activation of the HPA axis and cortisol release is anti-inflammatory and adaptive, prolonged and repeated stress exposure may lead to HPA axis exhaustion and cortisol dysfunction, including depletion, downregulation, and impairment of the negative feedback system<\/span><sup id=\"cite_ref-:4_85-2\" class=\"reference\" style=\"text-align: initial\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-:4-85\">[85]<\/a><\/sup><span style=\"font-size: 1rem;text-align: initial\">. Catastrophizing and other maladaptive pain beliefs may similarly lead to reactivation of the stress response, prolonged cortisol secretion and dysfunction. These changes ultimately result in reduced cortisol efficacy in the secondary, anti-inflammatory, stage of the stress response.<\/span><\/div>\n<\/div>\n<\/div>\n<\/div>\n<h3><span id=\"Cortisol_Dysfunction:_Effects_on_Pain\" class=\"mw-headline\">Cortisol Dysfunction: Effects on Pain<\/span><\/h3>\n<p>Cortisol dysfunction has been implicated in various inflammatory and autoimmune diseases, ranging from osteoporosis, rheumatoid arthritis, and chronic fatigue syndrome, among others<sup id=\"cite_ref-:4_85-3\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-:4-85\">[85]<\/a><\/sup>. HPA axis exhaustion and cortisol dysfunction can manifest as stress-induced hypocortisolism and are associated with musculoskeletal pain conditions including chronic low back pain and fibromyalgia. Additional studies have suggested hypocortisolism as a predictive risk factor for musculoskeletal pain<sup id=\"cite_ref-87\" class=\"reference\"><a href=\"https:\/\/physio-pedia.com\/Psychological_Basis_of_Pain?utm_source=physiopedia&amp;utm_medium=related_articles&amp;utm_campaign=ongoing_internal#cite_note-87\">[87]<\/a><\/sup>.<\/p>\n<p>Therefore, it is critical to address both pain and non-pain-related stress in treating pain. Such measures may include objective cortisol readings in a medical setting, or subjective self-reports of stress as part of a screening or physiotherapy evaluation. By identifying stress factors or maladaptive coping strategies early in the screening process,\u00a0<a title=\"Chronic Pain\" href=\"https:\/\/physio-pedia.com\/Chronic_Pain\">providers can educate patients about chronic pain<\/a>\u00a0and the interplay between psychosocial factors and pain. Physiotherapists may also utilize\u00a0<a title=\"Pain Neuroscience Education (PNE)\" href=\"https:\/\/physio-pedia.com\/Pain_Neuroscience_Education_(PNE)\">pain neuroscience education<\/a>.
